Clinical Observation of Peritonitis Caused by Perforation of the Terminal Ileum due to Salmonellosis

Abstract

|
Clinical observation was made for 34 cases of surgical patients who were operated between July 1967 and March 1968 at the surgical department of 59th Evacuation Hospital under the diagnosis of generalized peritonitis due to small bowel perforations which were suspected to be caused by the infection of salmonella.
Perforations were developed mostly (53.1%) at the 2nd and 3rd weeks from the onset of the diseases, and a majority of them (28.1%) were seen in 10 days.
Duration from perforation to operation were 17 patients (53.1%) within 24 hrs.
Although multiple perferation were encountered in some patients, 78.1% had only one perforation, and it¢¥s diameter was under 5 mm in 78%.
In 28 patients (87.5%) the site of perforation was within 20¡40 cm from the ileo-cecal junction.
The free air shadow of abdominal cavity by X-ray was positive in 28.1%.
The widal test was positive in 27 patients (79.4%), mortality rate after operation was 4 patients(11.7%) in 34 of surgical patients, and 8 patients (2.9%) of total 271 salmonellosis patients.
